FAILURE ANALYSIS:

  1. S/N 3 - functional failure after 500 T/C. C-SAM showed no TODD present. Decap revealed cracked passivation and further FA indicated presence of further ILD cracking with metal to metal shorting. FA report # IA030239. Corrective action implemented by fab site.
